the previous Qualifying examination. |

Certificate from the previous university / board
/ institution from where student attained qualifying
qualification. |

Transfer Certificate from the previous institution. |

Three
passport size photographs (including 1 to be pasted
on the form) with name of the candidate and mention
course applied for at its back. |

Self
attested copies of testimonials of all the qualifications
required for credit transfer, if the benefit of
Credit Transfer is desired by the candidate as
per Credit Transfer Scheme of the University. |

Attested
copy of (SC/ST, OBC, PHC, etc.) certificate from
appropriate authorities if seeking admission under
any of the reserved categories. |
